Race/Ethnicity: 這個是問種族, 我們選: Asian of Pacific Islander

Social Security Number(SSN) : 這個是美國境內(nèi)用的身份證號,不填

Country of Birth:

Birth City: 選CHINA

Country of Citizenship: 選CHINA

If not a U.S. citizen, are you a Permanent Resident of the United States?: 選NO

一般有幾個表, 一個是Mailing Address, 一個是Permanent Address, 有的還有工作地址什么的. 如果都是一個地址, 就不用天后面了. (if different from mailing address,才填).

Street:

City:

State/US Territory/Possession: 這個是選在美國哪個洲, 不管.

Province or Country(Non USA/Canada only): 省

Postal Code:或ZIP: 就是中國的郵編

Telephone: 這里一般留的是美國的數(shù)位(xxx-xxx-xxxx), 中國的擠不下,就不填了. 如果沒有數(shù)位限制, 就填上自己的電話.(Telephone Number Country Code: 中國填86), 比如北京的就是: 86-10-xxxxxxxx.

另外還有Other Contact Information, 比如EMAIL, FAX等.
